Add attachments to custom questions on forms

For example, I need my registrants to accept a liability waiver and I'd like to be able to attach the waiver to a question on my form.

Voting to add feature to allow user to review and accept a waiver. This could be done by allowing a scrollable PDF to be inserted along with a checkbox requiring the user to indicate "I have read and hereby accept the waiver," perhaps requiring e-signature and date.

Hello, it would be interesting to be able to add personalized forms when registering for events (e.g.: health check, recognition of risks and responsibilities (adults and minors), authorization to take photos/videos for promotional purposes). We are currently organizing a benefit ultra-trail for our organization and it would be easy for the registration platform to manage these functionalities as well.

Overview:
Many schools across the United States host events that are free of charge—such as field days, assemblies, or club activities—but still require signed waivers from parents or guardians for liability and participation purposes. Currently, this process is handled separately from event registration, often through paper forms or third-party tools. Integrating a digital waiver feature directly into Zeffy would streamline the process for both school administrators and parents.
Proposed Feature:
Allow event organizers to attach a digital waiver to any event.
Automatically send the waiver to the parent/guardian email provided during registration.
Track whether the waiver has been signed and store the signed status within the participant’s registration record.
Optionally, prevent completion of registration until the waiver is signed.
Benefits:
Simplifies the event registration and waiver collection process for schools and HSAs (Home and School Associations).
Enhances compliance and reduces administrative follow-up.
Provides a clear, auditable record of signed waivers.
Increases the appeal of Zeffy for school-related events, particularly for events such as proms or dances that currently rely on other platforms due to their waiver or signature capabilities.
Strategic Opportunity:
If this waiver feature is implemented, Zeffy could become the go-to platform for all school events—both free and paid. Schools and HSAs already using Zeffy for fundraising would have one integrated tool for both donations and event management. This could lead to school districts preferring Zeffy over competing apps, such as Dance School App, especially for high-profile events like proms.

In order for students to participate, parents need to complete a waiver. We currently house these waivers in a separate platform that is not compatible wiht Zeffy and it is a huge lift (and major pain point) to cross reference these two platforms ahead of any event. It would be phenomenal for Zeffy to house waivers and be able to indicate whether a student has a waiver on file as part of the guest list OR better yet, a family is unable to purchase a ticket until the waiver is signed.

It would be great to have an option to add a signature in forms, especially when the event involves minors or if you want add a waiver to the form instead of creating it somewhere else.

I would like to add a waiver PDF to the Campaign Event and ask the question
"I have read the waiver and agree to the terms and conditions in the Waiver."

This would be very helpful. I have posted here, on this topic before but, now that our program has started, my organization is second guessing use of Zeffy. The inability to be able to link payments to attachments here has really impacted our workflow. We are considering whether the fee saved is worth the extra time and lots of effort that we are putting in.

I'd like to be able to include policies or terms of use that people would have to read and agree to before signing up for their membership. I would prefer a pop-up with the terms that they would have to scroll to the bottom of before clicking that they read and agree to the terms. If not a pop-up, a page that that they would have to scroll through before going to the final check-out page.
This feature would also be ideal as an option for event tickets with liability waivers.

We need folks to agree to our photography release form for our event. The limited field types does not allow for this, and also does not seem to allow and urls so I can link to the form they must agree to.
