Add attachments to custom questions on forms

For example, I need my registrants to accept a liability waiver and I'd like to be able to attach the waiver to a question on my form.

I would like this feature. I need my registrants to send a pdf file of their sponsorship ad.

While this would be handy, we usually handle online waiver forms with JotForms. We like having all the documentation about the signer, the date and time, etc.
