Change "Buyer Details" to "Member (or donor) Details" depending on the form

Suggests that my members have to PAY for something but I am offering a free membership.

Add the word "Other" within the open field dollar amount box

This way donors know they can enter their own amount in the $0.00 box rather than select a premade button amount. Once they click a premade button it will default to the selected amount.

Add decimals ($0.00) to the custom donation amount box.

Currently it's the only box with different formatting. Make it unified with the rest of the numbers.
